$385.00 (includes 40 hours of ice, jersey and socks)

Notes:

A Parent or Guardian MUST BE PRESENT AT REGISTRATION to sign the Registration Form, the USA Hockey Waiver, and the Medical Insurance Forms

No SMAHL member will be closed out 

No mail in registration
